WAIVER & CONSENT FORM
TERMS AND CONDITIONS OF REGISTRATION
I hereby release and hold harmless Kingdomcity, its staff, volunteers and agents of liability in the event of an injury, accident, misfortune, damage or loss that may occur to me and/or my property/child while present at the Kingdomcity Kids program and within the program premises. The program experience will be captured in photographs, video and audio.
INVOLVEMENT CONSENT
I hereby give my consent for the registrant to participate in activities they may choose while attending the program. I agree and understand that Kingdomcity reserves the right to exercise its discretion to refuse to register any person upon medical and/or other grounds, without providing a detailed reason for doing so.
MEDICAL TREATMENT CONSENT
I give permission for Kingdomcity authorised staff and any volunteers to obtain emergency medical, hospital or ambulance assistance for myself and my family members at anytime they consider necessary. I understand that every effort will be made for me myself or my emergency contact listed above to be notified before instituting such processes. I acknowledge that I will be liable for any and all medical/hospital/ ambulance costs and expenses incurred in this treatment. I also understand that while every reasonable precaution will be taken to ensure practical protection, I hereby release and hold Kingdomcity staff and volunteers from any and all liability in the event of any injury, accident or misfortune, damage or loss that may occur to me/my child and our property while present at the Kingdomcity event or program.
MEDIA WAIVER
Occasionally at Kingdomcity, Fearless Royalty, Young Royalty and Baby Royalty will be involved in activities including media. The event experience may be captured in photographs, video and audio. Kingdomcity reserves the right to use the material for promotional purposes. Please understand it is a condition of the children/s registrants entry today into the event that all claims are waved in relation to inclusion of their likeness in such films, images or recordings.
FOOD & ALLERGIES
Whilst every effort is always made to avoid serving food containing allergens or traces of nuts, we are not, however, the manufacturer of these products and therefore parental consent is required so that all parties are aware and understand that if a person ingests food in one of our programs, they do so at their own risk.
It is the responsibility of all parents and/or guardians to ensure that any food allergies are communicated to Kingdomcity Kids and visible on the child's name-tag.
PRIVACY & DATA
This form once completed by you with any information ("data"), provides Kingdomcity (defined in our Privacy Notice) with a legitimate interest to hold and maintain such data according to Kingdomcity’s Privacy Notice, available at www.kingdomcity.com and confirms your consent (until revoked) for further contact.
